Disabled all other extensions, refreshed page, restarted browser a few times, refreshed page a few more time, etc... It didn't load a single time.

How do you know it "didn't load"? Did you check if the extension appears in the address bar addons? Can you check the browser console for any errors when on twitch.tv?
