First was seeing "Timeout or not connected to KeePassXC" as well, but manually enabling "Proxy" and "Native-Messaging updates" in advanced settings for "Browser Integration" allowed me to connect Keepassxc browser to keepassxc desktop on macOS 10.13.3.
