This might be the one extension/app tht really works for me to keep focussed on doing my job. It's in your face and just customizable enough to make the right intentions.

I've tried every blocker on the planet it seems, but this is different: it shows you that you CAN override it if you want, but it reminds you of your goals while you're doing it, like an encouraging / disappointed parent. Even though you can "disable" this far more easily with a single click, you won't because it reminds you to be your best self.
