Implement simplified icon; add HTML format and Ctrl+click option.

Unicode characters in URLs will be decoded by default (for example, 茶 instead of %E8%8C%B6). If you prefer the encoded version, there's a checkbox for that on the add-on's Options page.

(1) Added a new toolbar button to copy the page URL (if you're not interested, right-click > Remove from Toolbar; to position near the address bar, use the Customize feature).
