Fix bug with LiveVox calls not being correctly attributed to a state.

Fixed bug in FF 47.0+ where browser_action key suddenly had unrecognized attributes ("name" and "icons").

Fixes bug where switching services to VI (e.g. from NJ to VI) would result in VI calls being marked as going into the previous state (NJ in this example).

Fixed bug where VI calls weren't being recorded due to no ZIP being present.

Fixes bug where ZIP codes were incorrectly being parsed, due to LiveVox reporting unexpected information in the wrong location.

Bug fix that was making the content script fail silently, due to an undefined function check failing. This bug wasn't present in Nightly, but was in Firefox.

Change log:
- Add auto-auth for easier LiveVox logging in (currently inactive)
- Add workaround to make Guam calling trackable
- Add refresh button
- Switch to cookie checking for login status updates
- Major code refactors (no side-effects noticed in testing)

This addon now requires Firefox 45.0 because it makes use of browser.cookies (see https://developer.mozilla.org/en-US/Add-ons/WebExtensions/API/cookies/set).

Fixes two bugs:
* Addon wasn't able to communicate with server due to CORS and cookie issues.
* Profile links on popup did not open in new tab when logged in.
