Fixed 12h clock (AM/PM) handling for midnight (12 am is 00:00) and for times in the afternoon written as both a 24 clock time, and "PM".

Add on operating mode / status is now indicated and toggle-able on the browser action button.

== New Feature

The "browser action button" now temporary enables "insert conversions everywhere" behavior for the current tab. Clicking again turns it back off. This is useful on site that pop up times in tool tips, such as Azure graphs.
