I found what was causing the issue with this extension only working intermittently. It was another extension called "Don't Accept image/webp", which is designed to stop sites serving up WebP images. With both extensions enabled at the same time, sometimes the Reddit images extension would stop working, but would work again if you disabled and re-enabled it. If the WebP extension was disabled, the Reddit one worked perfectly.
I have added an exception for Reddit in the WebP extension and now there are no conflicts and both extensions function fine.

If you close your browser and re-open it, the extension no longer works until you disable, then re-enable it.
The functionality is great, but if you could fix that issue, it would be perfect.

(Okay, so technically I've since switched from this extension to a browser userscript that does the same thing, but this one works identically for solving the problem. Oh and BTW if any of you prefer that option, there's an issue in this project's Github repo that contains a working one.)
The problem this extension fixes is so annoying. Even after you click the image and Reddit forwards you to preview.redd.it, it was still practically impossible to display the raw image in a browser tab without downloading it to disk first! (And doing so is practically required if you want to zoom in, since the Reddit photo viewer's built-in zoom functionality is next to useless.)
