Improve link cleaning detection and drop heuristics

Detection is improved by detecting redirect loops and unencoded URLs inside the URL, identifying top-level URLs better, and also identifying non-cleaned links better.

Also drop redirects that contain parent URL, instead of anything non top-level, and maintain parameters on non-cleaned links.

Clarify & improve option to only clean top-level documents or all of HTTP traffic.
