Reorganize Header and Display Settings

New:
1. Review workflow: Mark files as viewed, track your progress, queue multiple comments, and publish one review with Comment, Approve, or Request changes.
2. Open in a separate tab: Open any pull request in its own diffy tab from the overlay, or right-click a PR page or PR link on GitHub and choose Open in diffy.
3. PR sidebar: See file, line, and comment counts plus branch info, author, status, labels, and reviewers at a glance.
4. Collapse files: Expand or collapse individual files from the file header. Viewed files collapse automatically.
5. Refresh: Reload the latest PR data after new commits land.
6. More themes: Expanded theme options with a refreshed header layout.
